The following criteria are used to determine eligibility for SJH HospiceCare services:

• Residency in Cumberland or Salem County or portions of Atlantic, Cape May and Gloucester counties.
• Diagnosis of any illness with a life expectancy of six months or less.
• Choice by patient of care coordinated by hospice that utilizes methods of pain and symptom control, which enable the patient to live as fully, and comfortably as possible.
• Consent of the patient's attending physician and certification by the hospice medical director.
• Our services are provided without discrimination. Patient and families who meet the eligibility requirements will have impartial access to hospice services without regard to age, gender, nationality, race, creed, disability, sexual preference, diagnosis, availability of a primary caregiver, or ability to pay.
