SJH Community Services is the exclusive provider of the HomMed^®Health Monitoring System, which collects patients’ vital signs on a daily basis. With this system, we are able to place hospital-grade monitors in patients’ homes to gather clinical data regarding trends in their health status. The data is reviewed daily by a registered nurse.

When patients have their vital signs taken seven days a week with home monitoring devices, irregularities can be caught before they become problems. Nurses can be sent to patients’ homes when they are needed, and doctors can adjust medication and diet right away based on patients’ daily data.

Early detection of problems can lead to early simple interventions that can eliminate the need for emergency room visits or hospitalization. Patients become more aware of the effect ofdiet, activity and other factors on their condition. Monitoring is a free service to our skilled care patients.

Active Seniors can also benefit from daily in-home monitoring.
