Summary:
This program is a comprehensive educational offering, designed to help people with diabetes.

Description:
Helps individuals with diabetes to better understand and manage their diabetes and related health problems.  It includes an assessment, an individualized meal plan, exercise recommendations, medication information and the setting of safe target ranges for blood sugar testing.  This program is offered in both group and individual (one to one) formats, and is recognized by the American Diabetes Association as "meeting the national standards for Diabetes Self-Management Education."
